The only thing that changed over the years is the cap has a hole at the end of it in case it gets stuck in someone's airway, the air can flow through it. Maybe meant for children or people who put things in their mouths? Safety feature done years ago.

That classic clear hex tube pen is the best when you need something inexpensive to write with.
